Hire and fire mentality which negatively impacts staff’s spirit, no clear strategy or differentiator from competitor, too many projects at once.

Treat your employees as you would like to be treated yourself. Take employee’s health really seriously (working without breaks will make more people ill). Invest more in (IT) personnel.
